Occupational Therapist

Are you ready for a new challenge or an improved contract? We have an excellent locum opportunity for an Occupational Therapist with solid experience working within a Discharge Team.

You will have a varied caseload, providing social care through an Occupational Therapy lens for adults with ongoing social care needs. This includes investigating safeguarding concerns, carrying out statutory assessments, commissioning support and presenting your reports to panels or in court.  You will also work with partner agencies, carers, and families to build networks of support that can help people maintain their independence.

We have excellent relationships with local partners and hospital trusts. Together we work collaboratively to serve people who have recently been discharged from hospital back to their home in the community or to a temporary residential setting. We promote the home first model and discharge to assess approach and believe with the appropriate safe level of care clients can maintain their independence and stay at home safely where possible. We work closely with our reablement teams to maximize independence and build confidence and positive outcomes for the adults we work with.  Where necessary we work with our local Integrated Commissioning Boards (ICB’s) to ensure the people we serve get access to continuing health care funding.

It is important to note that the role will include assessing adults in line with the Care Act which will include recommending referrals for provision of equipment and adaptations to enhance independence and reduce risk. The role will involve collaborative working to support our multi-disciplinary approach.

As an Occupational Therapist, you will have a pivotal role in the well-being agenda and inform the interventions needed to meet clients' and carers assessed eligible needs. You will manage and reduce risks, supporting safety in the community.
