Senior Nurse Assessor – Children and Young People (CYP)

 

The Senior Nurse Assessor – Children and Young People (CYP) is responsible for improving outcomes for children and young people with complex physical, mental, emotional health needs and/or learning disabilities. The role focuses on assessing, reviewing, monitoring, and supporting high-quality care packages, placements, and treatments through continuing care and specialist placement pathways.

The post holder will work across health, social care, education, and commissioning systems, providing expert clinical assessment, advice, and leadership. They will support children, young people, and families through complex care journeys, including transitions to adult services, while contributing to service development, policy, and quality improvement.

 

Location: Hybrid (based in Kent 2 days face 2 face visits required)

Job Type: Temporary 

Duration of booking: Expected to last 3 months with possible extension

Proposed start date: ASAP

Pay Rates: Up to £310 per hour Umbrella

Hours / Working Days: 37.5 hours per week / Monday to Friday, 9am – 5pm

Key Responsibilities

 

Clinical and Professional Practice

  • Undertake specialist continuing care assessments to determine eligibility for care packages
  • Review and monitor existing care packages, placements, and treatment arrangements
  • Carry out risk assessments and implement effective risk management plans
  • Provide clear, evidence-based clinical advice to children, young people, families, carers, and partner agencies
  • Work collaboratively with families and multidisciplinary partners to ensure care is person-centred and as close to home as possible
  • Support the implementation and monitoring of Personal Health Budgets
  • Contribute to Care, Education and Treatment Reviews (CETRs) for children and young people with complex needs
  • Support children and young people throughout transition phases, including transition to adult services
  • Work within continuing care and specialist placement services across a wide geographical area
  • Liaise with health, social care, education, and commissioning partners at local and national levels
  • Support children and young people requiring community-based or inpatient care
  • Act as a professional resource for multi-agency partners regarding complex CYP cases
  • Promote safeguarding and uphold best practice in line with statutory and professional standards

 

Leadership, Management and Development

  • Support the Lead Nurse in service delivery, policy development, and protocol implementation
  • Contribute to performance improvement initiatives and service development projects
  • Monitor and manage risks, issues, and action plans within agreed workstreams
  • Support effective information management, reporting, and governance processes
  • Contribute to budget oversight and value-for-money decision making
  • Assist in developing and monitoring key performance indicators
  • Contribute to the development and review of policies, procedures, and care pathways
  • Participate in research, audit, and service evaluation activities
  • Support continuous improvement through evidence-based practice

 

Person Specification

 

Essential Qualifications

  • Registered Clinician with current professional registration (Children’s Nursing or Learning Disability Nursing)
  • Degree or diploma level qualification with relevant specialist training
  • Evidence of ongoing post-registration professional development
  • Essential Experience
  • Significant experience working with children and young people with complex health needs
  • Experience of assessment, case management, and reviewing care packages
  • Experience working in multi-disciplinary and multi-agency environments
  • Knowledge and application of national continuing care frameworks
  • Experience supporting children and young people with learning disabilities
  • Experience of managing risk and working in complex, sensitive environments
