The Role - 15 Months 

Calling all amazing Social Workers! Are you passionate about making a real difference in the lives of individuals with complex needs? This role is for you! We’re looking for dedicated, driven professionals with experience in Continuing Healthcare (CHC) or complex care pathways who want to bring their compassion, problem-solving skills, and heart into every case.

You’ll be working as part of a supportive multidisciplinary team, helping people get the care and support they need to live safely and independently. If you’re confident with eligibility assessments, know your way around the Care Act, and thrive in a fast-paced environment, we’d love to hear from you.

What You’ll Need

Registered with Social Work England Experience supporting adults with complex needs or in CHC settings Strong knowledge of the Care Act & Mental Capacity Act At least 6 months’ experience in a UK social care role Right to work in the UK + UK-based references
