Role Overview
As a Specialist Nurse within our Clinical Operations team, you will play a vital role in delivering safe, effective, and patient-centred care directly in the community. This position is designed to prevent unnecessary hospital admissions by providing high-quality clinical interventions in a home setting.
You will combine expert clinical practice with independent decision-making, working as part of a multidisciplinary team to support patients during a crucial stage of their care journey. This role is ideal for nurses passionate about community-based care, continuity of service, and building meaningful patient relationships.
Location: Milton Keynes and surrounding community areas. Focus: Acute care delivery and hospital admission prevention.
Core Objectives and Responsibilities
Your focus will be on maintaining clinical excellence and patient autonomy through the following actions:
- Acute Clinical Delivery: Administering sub-cutaneous injections and IV therapies, including the comprehensive care of IV devices.
- Community Support: Working autonomously in the community to support patients at home, ensuring safety and clinical efficacy.
- Clinical Supervision: Providing confident supervision to others within the multidisciplinary team to ensure high standards of care.
- Service Adaptation: Managing a dynamic workload with an organised and adaptable approach to meet the evolving needs of the service.
- Patient Empowerment: Supporting the mission to give patients control of their health through knowledge, choice, convenience, and connection.
Candidate Requirements
To be successful in this specialist community role, you must possess the following qualifications and experience:
- Registered General Nurse (RGN) with current NMC registration.
- Post-registration experience specifically within an acute care setting.
- Technical Proficiency: Proven experience in administering IV therapies and sub-cutaneous injections.
- Clinical Independence: The ability to work autonomously and make informed clinical decisions in a field-based environment.
- Strong Communication: The skills required to work closely with patients and families in their own homes.

- Starting Salary: From £34,000 per annum.
