Job Summary
We are seeking an experienced and motivated Verification Nurse to join the Continuing Healthcare (CHC) team. The successful candidate will be responsible for verifying CHC and Funded Nursing Care (FNC) assessments to ensure compliance with the National Framework for NHS Continuing Healthcare and local commissioning policies.
You will provide expert clinical oversight, ensure quality and consistency in decision-making, and support the CHC team in maintaining robust governance and audit standards. This is a key quality assurance role, requiring strong clinical judgement, analytical skills, and attention to detail.
Key Responsibilities
Clinical Verification & Governance
- Review and verify CHC and FNC assessments, ensuring eligibility decisions align with the National Framework.
- Ensure quality, accuracy, and consistency across DSTs (Decision Support Tools), checklists, and review documents.
- Provide clinical advice and feedback to assessors to maintain a high standard of submissions.
- Identify and escalate complex or borderline cases to the CHC Clinical Lead or Verification Panel.
- Contribute to internal quality assurance processes and continuous improvement initiatives.
Communication & Coordination
- Work collaboratively with CHC Nurses, Social Workers, and MDT members to ensure accurate evidence gathering.
- Attend verification meetings and panels as required, offering clear and well-evidenced recommendations.
- Communicate outcomes sensitively and professionally with patients, families, and partner organisations.
- Support effective liaison between NHS, local authorities, and independent providers.
Record Keeping & Compliance
- Maintain accurate, confidential, and auditable records in accordance with Information Governance standards.
- Ensure all activity complies with NHS Continuing Healthcare policies, procedures, and equality legislation.
- Contribute to audit reviews and data quality reporting.
Professional Development
- Maintain current NMC registration and professional competencies.
- Participate in supervision, reflective practice, and training sessions.
- Support colleagues through mentoring and sharing best practice.
Essential Criteria
- Registered Nurse (Adult, Mental Health, or Learning Disabilities) active NMC registration.
- Minimum 3 years’ post-registration experience within CHC, community nursing, or similar setting.
- Strong knowledge of the National Framework for NHS Continuing Healthcare (2022).
- Proven ability to review clinical evidence and make sound, defensible eligibility recommendations.
- Excellent written communication and report-writing skills.
Desirable Criteria
- Previous experience as a CHC Assessor or Verification Nurse.
- Knowledge of FNC processes, PHBs (Personal Health Budgets), and complex discharge pathways.
- Experience participating in verification or appeals panels.
